Translation / modern text

[Upper margin on the right:] Consistent Formula of the oath I, Mikołaj, Archbishop of Lviv, from this hour forward will be faithful and obedient to blessed Peter and the holy Apostolic Roman Church and to my lord, Pope Innocent X, and his canonically entering successors. I will not take part in any counsel, nor will I give consent or contribute by deed to them losing life or limbs, or being captured in evil captivity. The counsel which they entrust to me personally, through messengers, or by letter, I will knowingly reveal to no one to their detriment. I will be helpful to them in preserving and defending the Roman papacy and the regalia of Saint Peter against every man. I will treat the Legate of the Apostolic See with respect on his journey to and fro and assist him in his needs. When summoned to a synod, I will attend, unless I am hindered by a canonical impediment. I will visit the Apostolic Thresholds every five years personally or through my messenger, unless I am released by apostolic grace. I will not sell, give away, pledge, re-grant as a fief, or in any way dispose of the goods belonging to the endowment of my archbishopric's table without seeking the counsel of the Roman Bishop. I will also observe the constitution prohibiting the investiture of jurisdictional goods belonging to lower churches, issued in the said Year of Our Lord one thousand six hundred and twenty-five. So help me God and these holy Gospels of God. [Signatures / attestations on the left side:] Hugo de Metis, protonotary [?] [N.
