WŁADYSŁAW IV, by the grace of God, King of Poland, Grand Duke of Lithuania, Ruthenia, Prussia, Masovia, Samogitia, Livonia, Smolensk, and Chernihiv, and also hereditary King of the Swedes, Goths, and Vandals.
We hereby announce to all and each one whom it may concern. A paper document was presented to Us, extracted from the books of the Greater Metrica of the Crown Chancellery, bearing a seal, containing the privilege of the once Most Serene Casimir, King of Poland, granted for the customs duty in the village of Kościejów in the Lviv land, sound, whole, intact, and free from any suspicion, and We were requested to approve and confirm it with Our royal authority. The wording of which follows as such:
WŁADYSŁAW IV, by the grace of God, King of Poland, Grand Duke of Lithuania, Ruthenia, Prussia, Masovia, Samogitia, Livonia, and also hereditary King of the Swedes, Goths, and Vandals. We hereby announce to all and each one whom it may concern. The document was found in the current records of Our Greater Chancellery and We were requested to order its authentic issuance. The wording of this document follows word for word and is as follows:
In the name of the Lord, amen. In eternal memory of the matter. Casimir, by the grace of God, King of Poland, Grand Duke of Lithuania, Lord and heir of the Ruthenian, Prussian, Chełm lands, etc. We announce by the wording of this document, to whom it is due, to all and each one, both present and future, who shall receive knowledge of this. That although the care of the royal majesty should be directed toward the benefits of its people, a small part of the dowry of piety should all the more flow to those whom the hand of the Lord has touched and who, abandoned by all in this world, await a reward. Citizens strengthened by generosity may devote themselves to their contemplations and with greater piety implore the benevolence of the Highest for Us and Our successors. And how useful are the benefits from customs duties for Us in this world, so that in the future we may possess a saving reward for them with multiplied fruit! Therefore, wishing to anticipate the day of the final judgment with works of piety and to exchange earthly things for eternal ones, the customs duty in the village of Kościejów in the Lviv land, amounting to half a groschen of current coin from every loaded wagon of every merchant passing through it, with the unanimous consent of Our then-present starosts [?] and at the request of the Lviv citizens, who reported that travel through there cannot take place without heavy and significant danger and road repair – we have raised and established, and we have given and granted that customs duty to the church and convent of Corpus Christi of the Preaching Brothers in Lviv, and we give, grant, and incorporate it for eternal times by the power of this document, to be collected by the aforementioned conventuals or their seniors for the maintenance of the convent at the castle. So, however, that by reason of our aforementioned donation, the brothers or their current prior should take care [...]. Which eternal grant [...] we strengthen [...]. Given in Bełz, on Monday, on the very day of St. Maurice the Abbot, in the Year of the Lord one thousand four hundred and seventy-sixth. In the presence of the illustrious: Dobiesław Rożyc [?] of Kurozwęki, Castellan of Lublin, Spytek of Jarosław, Voivode of Ruthenia, and Stanisław called Wątróbka of Strzelce, Voivode of Bełz, Dobiesław of Byszów, Castellan of Bełz, Rafał of Jarosław, Chamberlain of Przemyśl, and [?] Chamberlain of Lviv. Given by the hands of the noble Stanisław of Kurozwęki, Deputy Chancellor of the Kingdom of Poland.
Kindly acceding to this request, we have ordered the aforementioned document extracted from the records of Our Chancellery to be issued authentically, wishing that it always retains its due force. In testimony of which, the seal of Our Kingdom has been attached to this document. Given in Warsaw, on the 16th day of February in the Year of the Lord MDCXLV, in the 13th year of Our reign in Poland, and the 14th in Sweden. Łukasz Opaliński, Chancellor of the Kingdom of Poland.
We, therefore, King Władysław, led by the request [...], approve, confirm, and ratify the above-included document in all its points [...]. Given in Warsaw, on the 18th day of March in the Year of the Lord MDCXLVI, in the 14th year of Our reign in Poland, and the 14th in Sweden.
Władysław King, by own hand.

Parchment document in good condition, with visible traces of folding and a water stain in the lower part. The seal has not survived (cut off/torn off); only a fragment of the braided seal cord in red and white/silver colors remains. The document is a confirmation by Władysław IV of an extract from the books of the Crown Metrica (issued in Warsaw on February 16, 1645, by Chancellor Łukasz Opaliński), containing the privilege of King Casimir IV Jagiellon issued in Bełz on September 22, 1476.
