A clean sheet of parchment (blank form in blanco) with the seal of the Metropolitan of Kiev, Michael Rohoza, attached on a green silk cord in a turned wooden bowl (skippet). In the bottom left corner, an annotation with a signature/paraph: "рꙋка бланкатꙋ". Dating based on the years of Michael Rohoza's tenure as Metropolitan (1589–1599). Former signature of the Library of the Shevchenko Scientific Society in Lviv: no. 1065.
