The mayor and councilors of His Royal Majesty's town of Kałusz
We announce by this our certification that we have been delivered a universal of His Royal Majesty, our Gracious Lord, regarding a donation, or rather a contribution, of one hundred thousand zlotys for the current need of His Royal Majesty, our Gracious Lord, and the Commonwealth, with which universal a letter from their Graces the Lviv Councilors was also delivered to us. For which we give this our certification with the impression of our council seal. Given in Kałusz on the 17th day of April in the year 16[...]

Reading notes
Discrepancy with the Polish inventory description: the inventory incorrectly lists Kalisz, whereas the document was undoubtedly issued by the mayor and councilors of the town of Kałusz (in Red Ruthenia; the Ukrainian description correctly states the town of Kałusz). The end of the year on p. 2 is obscured by paper under the seal, however, a clear chancery date has been preserved on the reverse (p. 3): 1629.
