A partly legible, formal letter to the Lviv magistracy. The writer describes a matter and asks that it be considered or acted upon; a reference to Warsaw and officials can be made out, but the details remain uncertain. The letter has no reliable date.

[Nagłówek łaciński:] „Spectabilis ac Excellentissime Domine [...] mihi observandissime”.
[Tekst s. 2:] Długi, częściowo czytelny list po polsku. Widoczne fragmenty obejmują wzmiankę o „[...] do Warszawy [...]”, osoby/urzędników oraz prośbę o rozpatrzenie lub załatwienie sprawy; wiele zdań i nazw własnych jest niepewnych.
[Podpis:] prawdopodobnie „Wojciech Zimnicki” [odczyt niepewny].
[Data:] brak pewnego datownika w treści.

Reading notes
Page 1 is an inventory cover; its date was not used as the letter date. The document itself is on p. 2, while p. 3 is a folded address sheet with a seal to the Lviv magistracy. No reliable dateline is visible on p. 2, so the date and place fields are empty. The signature is tentatively read as Wojciech Zimnicki, but the reading is uncertain.
