In Ulchówek, on September 28, 1699
Most Illustrious and Gracious Lord and Benefactor!
Since the will of God has seen fit to make me so unhappy that, by the unexpected taking from this world of my beloved husband and benefactor of blessed memory, it has left me orphaned and burdened with heavy grief, I find so much comfort in my unbearable sadness that, after the Lord God [and] my parents, I choose the Most Illustrious and Gracious [Lord?] and my Benefactor as my primary guardian in my heavy orphanhood, just as in the Most Illustrious Your Grace, my Benefactor, I also have a guardian and benefactor after the Most Illustrious His Grace, my Benefactor. Turning to whom in this my orphanhood, I bring a humble supplication that you, Most Illustrious and Gracious Lord and my Benefactor, might deign to know about me, your orphan and servant; having commended myself to your protection and grace, I sign off.
[Bottom left:]
To the Most Illustrious Her Grace, my Benefactress, I humbly offer my submission at her feet, recommending myself to the grace of both of you, my Benefactors, that you might wish to know about me, an orphan.
M. 2.
[Bottom right:]
Of the Most Illustrious [Your Grace / His Grace?] Lord and Benefactor, the lowest servant,
Teresa Madalińska, wife of the standard-bearer of Wieluń
[P. 3, address:]

On p. 2, the date '28. 7bris 1699' is visible (incorrectly read as September 27 in the inventory card). The author's deceased husband was the standard-bearer of Wieluń from the Madaliński family.
